Home and Community Local Health Integration Networks (formerly known as Community Care Access Centres or CCACs) provide information and referrals to a wide range of community health and support services. For local branch offices, refer to the region’s website.
The GTA Rehab Network's Rehab Finder is a searchable database of rehabilitation programs/clinics/services provided by hospitals and Community Care Access Centres (CCACs) across the Greater Toronto Area.
Health Care Connect
