Referral Forms

In an Outpatient Clinic or Day Hospital

A person may be referred to a day hospital or outpatient clinic for more rehabilitation after they leave the acute, community or rehab hospital.

The person will live at home during this stage and come into the hospital or clinic for scheduled therapy appointments. Depending on their needs, the person will see a physiotherapist, an occupational therapist, a speech and language pathologist, a social worker and/or other healthcare providers as needed.

Treatment is offered approximately 2-3 times a week. The focus is to improve independence. This may also include a return to work or school.

In Toronto, outpatient ABI rehabilitation services are offered at the following rehabilitation hospitals: Holland Bloorview Kids Rehabilitation Hospital, Bridgepoint Sinai Health System, Toronto Rehab/UHN and West Park Healthcare Centre.

Who pays for rehabilitation?

The cost of rehabilitation services may be covered by the Ontario Health Insurance Plan (OHIP), the Workplace Safety and Insurance Board (WSIB), automobile insurance, or private disability insurance. The costs associated with Inpatient rehabilitation are most often covered by OHIP.
