Accessing Community Support Services
Questions about community Support services for your clients?
Join us March 7 at 12 noon for an overview of community support services available in the GTA, including outreach, case management and resource management. Moderated by the Toronto ABI Network, the webinar will be presented by staff from Community Head Injury Resource Services (CHIRS), Cota, March of Dimes Canada, Mackenzie Health & March of Dimes Canada – York Simcoe Brain Injury Services, Mind Forward (previously PHDABIS) and PACE Independent Living.
Who should attend?
Professionals in health care and/or social services in hospital and community organizations that work with individuals that have an acquired brain injury. In particular, those involved in care coordination and/or discharge planning.
