Across the Care Continuum: Community-Based Services & Support for Individuals with an ABI (October 31, 2014)

The Toronto ABI Network sponsored a half day information session on October 31, 2014 at St. John’s Rehab – Sunnybrook Health Sciences Centre to refresh knowledge on services and supports available to those living with an ABI in the community. The afternoon brought together 16 organizations and over 50 health care professionals working in acute care hospitals, rehabilitation hospitals, brain injury associations, and a variety of other community service agencies. The presentations focused on neuropsychiatry, behavioural outreach and support, outpatient rehabilitation, residential and ongoing community supports as well as survivor and caregiver support.
