Patients with acquired brain injury often struggle when they leave the hospital to return to life in the community. A well-planned and coordinated transition is critical.

Hospital to Community Transition Planning for Acquired Brain Injury (ABI): A Best Practice Guideline provides interprofessional teams with a clear set of best practices for the four stages of transition planning:

The guideline supports a specialized approach to hospital to community transition planning for individuals with brain injury based on established best practices and the needs of individuals with ABI and their families and caregivers.
