Network releases guideline for transition planning
The Toronto ABI Network has released its new best practices guideline for planning the hospital to community transition for individuals with acquired brain injury.
With many programs limited due to COVID-19, well-planned and supported transitions are more important than ever.
Hospital to Community Transition Planning for Acquired Brain Injury (ABI): A Best Practice Guideline provides interprofessional teams with a clear set of best practices for the four stages of transition planning:
- Early in the Hospital Admission
- During Inpatient Stay
- Closer to Discharge
- In the Community
The Network also maintains an up-to-date resource on current wait times for ABI community services to help clinicians, patients, families and caregivers plan transitions.
